ADSS provides service coordination to elderly and disabled individuals to assist them stay in their own home and community rather being institutionalized for care and services. ADSS offers premier customized services based on individual needs through case coordination, assessment, service development plan, referral, monitoring and remediation as needed. We are serving participants under Community HealthChoices, AmeriHealth Caritas PA health Plan.

Maximus – replace with Pennsylvania Independent Enrollment Brooker (Maximus)
- Get in touch with Pennsylvania Independent Enrollment Brooker (Maximus)
- PA Independent Enrollment Brooker will set up appointment with the client after the referral.
- PA Independent Enrollment Brooker assessor meets with the individual and completes an initial assessment.
- PA Independent Enrollment Brooker facilitates with the individual’s physician to obtain a Physician’s Certificate within 30 days.
- Once Physician’s Certificate is obtained, PA Independent Enrollment Brooker will make a request to the AAA to have a LOCA completed to determine medical eligibility for Waiver services (within 30 days).
- After LOCA is completed, PA Independent Enrollment Brooker will make a request to the local CAO to determine financial eligibility.
- PA Independent Enrollment Brooker offers the individual a choice of a Service Coordination entity to facilitate services (offered at Maximus’ initial visit).
- Upon state approval (Office of Long-Term Living), the Support Coordinator will meet with the individual and offer choices of direct care providers (Provider Agency Services/Home care Agency) and assist in facilitating any additional services the consumer needs.
