Aging and Disability Support Services is looking for Service Coordinators to provide culturally and linguistically relevant support coordination in the Capital/Lehigh region.

The service coordinator will assess for home care needs using Office of Long-Term Living and Managed Care Organizations guidelines. They will create, update and monitor the service plan and mitigate any issues to assist the person continue staying at home and community. Respnsibilities includes but not limited to: Complete a person-centered need assessment (InterRAI) and a level of care or need evaluation annually or as needed. Develop a Person-Centered Service Planning (PCSP) for each participant and revise at least quarterly. Review the participant’s needs, SMART goals and outcomes with the participant and their representatives timely. Complete at least one telephone call per month per participant and face-to-face visit quarterly or as needed. Offer provider choices for the services that are approved and authorized by the participant’s Managed Care Organizations and work with those providers to implement authorized services accurately. Document all services provided in appropriate databases. Coordination of services with service providers and critical incident reporting. Education: Must be a registered nurse or have a Bachelor’s degree in social work, psychology, or other related fields with at least three (3) years of experience in a social service or healthcare related setting. Bachelor’s Degree and one year of experience in Long-Term Support Services is acceptable. Additional skills: Proficiency in Microsoft Office, Excel. Knowledgeable on local, state and federal resources for low-income participants. Ability to pass clearances for Pennsylvania child welfare and required criminal history checks. This job requires working from home, office and traveling in a community. A dependable car is required. Preferences for someone who is bilingual in Spanish and with previous service coordination experience.
