ADSS is a service coordination entity under the Office of Long Term Living, HCBS, Community Health Choices. ADSS provides linguistically and culturally appropriate support coordination in capital Lehigh region. ADSS provides service coordination to elderly and disabled individuals to assist them stay in their own home and community rather being institutionalized for care and services. ADSS provides person centered services based on individual needs through case management, assessment, service plan, referral, monitoring and remediation as needed. We are serving participants under Community HealthChoices, AmeriHealth Caritas PA health Plan.

ADSS provides service coordination to participants who has Medical Assistance- Medicare and/or Medicaid, Community Health Choices.
ADSS strives to provide linguistically and culturally appropriate service coordination to consumers who have limited English proficiency, as well as mainstream American community. We know well how daunting and challenging impaired and physical disabilities and old age can be, but having right support services can help you achieve goals of staying home, with your loved ones, and experience an enhanced quality of life. ADSS works with you to develop individualized plans to best suit your needs, coordinate, recommend and connect services to assist you along the path to meet your everyday needs.
The elderly, physically challenged and differently abled (incapacitated) populations in Pennsylvania are growing. According to the recent United States Census (2016) projection, 17.4% of PA residents are 65 years and above and 9.5% of people under age 65 years are living with some forms of disability. ADSS serves those consumers growing old and disabled due to health ailment who requires support services to help them make their life easier.

ADSS provides service to participants of Medical Assistance and private insurance carrier individuals. We participate in the following waiver services- Aging waiver, Attendant Care/Act 150, Independence waivers program and Community Health choices.
